EIS Annual reporting form procedures

Please click here for the pdf file with instructions on completing the EIS  form required by all Independent Schools annually.

NOTE: The section of the  form regarding Basic French must be completed even if your school does not teach Basic French – if this is the case please mark “NO FRENCH” across the appropriate page of the form, sign and send accordingly.

Once you have completed to the end of page 8 in the procedures please follow these steps

  • print the one page report
  • read it over for accuracy
  • sign the document
  • mail the signed copy of the letter and the disc to the following;
